İBB Sosyal Hizmetler Daire Başkanlığı ve Hıfzıssıhha Müdürlüğü’nün Evde Sağlık Hizmeti 2001 yılında uygulanmaya başladı. Aslında bu hizmet, Dünya Sağlık Örgütü’nün Sağlıklı Kentler Projesi’nin kapsadığı bir hizmet; çünkü bu projede İstanbul’un da yer almasını Dünya Sağlık Örgütü istedi.

Hizmetin amacı gelir düzeyi düşük ve bakıma muhtaç hastaların günlük yaşamını kolaylaştırmak  ve hastalık ve tedavi sürecinin olumsuz etkilerini azaltmak. Sonda, serum, katater takmak, kan almak, ilaç uygulamaları gibi hizmetlerin yanında hastanın hipertansiyon, diabet, gebelik ile ilgili sağlık durumları takip ediliyor ve bazı basit tıbbi uygulamaları yapabilmeleri için hastaya, ailesine ve yakınlarına eğitim veriliyor.

 

Elbette hizmetten yararlanabilmenin şartları var. Evde Bakım Hizmeti, tüm İstanbullulara sunulmuyor. Yararlanmak isteyen hasta yatağa bağlı, yaşlı ve bakıma muhtaçsa, özürlüyse ya da hamileyse bu fırsattan yararlanabiliyor; ama ayrıca hiçbir sağlık güvencesi de olmamalı. Yeşil kart sahipleri, emekliler ve Sosyal Güvenlik Kuruluşlarına kayıtlı bireylerin bakmakla yükümlü oldukları kişiler de yararlanamıyor.

 

Nasıl kullanabiliyoruz?

 

Evde Sağlık Hizmeti’nden hizmet alabilmek için ilk başvuru Beyaz Masa hattını (153) arayarak ya da www.ibb.gov.tr’ bulunan Evde Sağlık Başvuru Formu’nu doldurarak yapılabiliyor. Talep, gerekli işlemlerin başlatılması için Sağlık ve Sosyal Ön Değerlendirme ekibine iletiliyor.

Tıbbi personel ve sosyal hizmet ekibinden oluşan bir ekip, hastaların sağlık ve sosyal ihtiyaçlarını saptıyorlar ve evinde muayene  yapılan hastanın nasıl bir tedaviye ihtiyacı olduğuna da karar veriliyor.  Böylece evinde muayene yapılan hastanın nasıl bir tedaviye ihtiyacı olduğu belirlenerek hastalığı takibe alınıyor ve hastalığın seyrine göre gerekli sağlık yönlendirmeleri yapabiliyor. Bir de mahallenin muhtarını çağırmak, ailenin gelir düzeyini ve sağlık güvencesi olup olmadığını belgelemek gibi detaylar var. İlk görüşme hem muayene hem de mülakat niteliğinde bir görüşme oluyor.

 

Yapılan muayeneler sonucunda eğer hastanın evinde tedavi görmesi yeterli bulunursa, reçete yazılıyor ve ilaçlar satın alınıyor ve sağlık personeli tarafından hastaya ulaştırılıyor. İlaçların kullanımıyla ilgili, hasta yakınlarına da mutlaka kullanım/uygulama bilgileri veriliyor. Evde tedavi olması mümkün görünmeyen hastalar için ise hastanede tedavi görmeleri sağlanıyor. İhtiyaç sahibi ailelere bu ilaçlar , tıbbi destek malzemeleri ve benzeri konularda sosyal yardım hizmetleri sağlanıyor. Ve bütün bunlar gerekli görülen periyotlarda tekrarlanıyor.
